Policies

Insurances Accepted

Our office accepts most insurance plans. Please call if you are not sure and we will be glad to assist you. Please bring your insurance card and a picture ID with you to your appointment.

Referrals

It is our policy to treat all illnesses and injuries as appropriate. Referrals are given at the discretion of the primary care provider. If you are referred to our clinic, be sure and verify with your insurance company that the provider is covered under your plan. If your plan requires you to have a referral prior to being seen by one of our providers, it is your responsibility to obtain the referral or we may not be able to treat you.

Prescription Information

When you need a prescription refill, we request that you give us 48 hours notice. Please do not wait until you run out of medication. Please phone your pharmacy for refill requests. If your prescription has run out of pre-authorized refills, they will fax a request to the office. This is the most efficient way to receive your refill medications. Occasionally, your physician may not automatically authorize a refill and will require you to make an appointment to be seen.

Payment Terms

Your insurance requires that you pay your co-pays at the time of service. If you do not have health insurance, we offer a prompt pay discount, provided you pay in full at the time of service.

We realize you expect us to provide you with quality healthcare and we will do this, in return, we ask that you honor your financial obligations to us and pay your portion promptly.
